Apply to be a court appointed special advocate volunteer
If you have already attended a Volunteer Information Session, and want to become a CASA volunteer, then this is the next step!
Please read below carefully before you begin. The form available via the linked button is SSL encrypted for security purposes.
You are about to complete the application for pre-service training. This is a lengthy form that must be completed in one sitting. You may take as much time as you like to answer the questions, but please note that there is no "save" feature.
To streamline the process, please note you will be asked for the following information that you will want to have prepared in advance:
Employment History
You will need your last 3 employers names, addresses and telephone numbers, along with the dates you worked there, and who your supervisor was.
Driving License and Insurance Information
You will be asked for your driving license number, issuing state, and expiry, along with your Auto Insurance company name and auto insurance expiry
References
You will need to provide 4 personal references, their names, their phone numbers and their email are required, and addresses are optional but good to have
Personal Statement
You will need to write 250 words (we count them!) on why you want to be a CASA (use Word or Google Docs to do write it and word count it, then copy and paste in when you get to that part) — there is no wrong answer, just tell us why you want to be a CASA! Your story will be kept confidential and only shared among CASA staff for screening purposes.
All questions MUST be answered
If a question does not apply to you, answer n/a. DO NOT LEAVE ANY QUESTIONS BLANK!
